Hospice RN Case Manager

Job Description 

RNs provide direct patient care in the patients home.  The nurse works with the IDG team comprised of the Physician, RNs, Social Worker, Chaplain, Volunteer Coordinator, Home Health Aides and Music Therapist to ensure our patients receive the highest quality of care.

RNs assess hospice patient eligibility, work with the marketing team to educate the community on hospice services and work with the patient and caregiver to ensure all of their needs are met.

 

 

Job Requirements

Qualified candidates must possess the following: 

• Registered Nurse; Nurses must legally reside in Idaho to be eligible to have a multi-state license
• Nurse Licensure Compact
• 1-2 years hospice or home health experience 
• Reliable transportation
