Job Description
Duties and ResponsibilitiesThe Transition Support Coach functions as a facilitator of interdisciplinary collaboration and care continuity across care settings. Interaction with the patient and care giver may occur at various touchpoints across the continuum, based upon assignments, beginning while the patient is in the hospital and extending to include follow up phone calls and potential visits to the patients discharge environment. This role is focused on provision of information and support for the patient and caregiver in order to ensure effective care transitions, improved self-management skills and enhanced patient--practitioner communication. Specific performance duties will be based on the population assigned to each Coach with expectations of being able to provide cross-coverage for each team members assignment. Some travel is expected.
Job Requirements
Qualifications- Bachelors degree in Nursing required
- Previous experience in Case Management, Care Coordination, home health, and/or managed care setting
- Current American Heart Association Basic Life Support for HealthCare Provider certification (BLS/HCP) required at time of hire
- Must have one (1) year nursing experience
- Must be computer literate, able to effectively utilize hospital systems, and learn new applications as they are introduced
- Must be familiar with reimbursement rules and regulations in the State of Idaho.
- Must have strong listening and communication skills
- Must be able to work collaboratively with multidisciplinary groups of providers
- Must be able to demonstrate leadership skills, independent problem solving, and decision making abilities
- Must be able to handle multiple tasks and problems simultaneously
- Must have a high level of initiative and the ability to self-direct

Must hold a valid Idaho Driver’s License

Preferred Qualifications
Masters in Nursing strongly preferred
Certification in health coaching preferred.
Shift:
Full-time, Days, 0800 - 1700 (however, must have flexibility in work hours as need arises)
